Title:Study of brain stem auditory-evoked potentials (BAEPs)and visual-evoked potentials (VEPs) in leprosy.

Abstract:A study of brain stem auditory-evoked potentials (BAEPs) and visual-evoked potentials (VEPs) was done on 25 newly diagnosed patients with leprosy whose diagnosis was confirmed by skin biopsy. The results were compared with 25 age- and sex-matched healthy controls. In BAEPs the important observations were the prolonged latency of wave V in 13 (52%), delayed interpeak latency (IPL) of wave I-III in 5 (20%) cases, of wave III-V in 12/25 (40%), suggesting a conduction abnormality of the VIII cranial nerve in its peripheral part, in its nucleus and in its connection in the brain stem. In VEPs, a delayed peak latency of major positive potential (P100) was seen in 20 cases (80%; 11/13, 84.6% TT; 7/10, 70% LL; 2/2, 100% BL), suggestive of subclinical optic nerve involvement. The BAEPs and VEPs were both abnormal in 10 cases (40%; 3/13, 23% TT; 5/10, 50% LL; 2/2, 100% BL). Conduction abnormalities of the central nervous system (CNS) were observed more frequently in lepromatous leprosy, as in other forms of peripheral neuropathy such as hereditary motor sensory neuropathy type I (HMSN I). There is a fair possibility of similar multiple demyelinating lesions in the CNS also, as is seen in leprous peripheral neuropathy. This hypothesis requires further strengthening by an extensive study of multimodality evoked potentials with magnetic resonance imaging in the patients. Histopathological and immunofluorescent studies of autopsy material of the brain can also contribute significantly to solve the dilemma. (AU)^ien.

Title:Minocycline in Lepromatous Leprosy.

Abstract:Twelve patients were treated with three dose levels of minocycline for 30 days, primarily to detect the dose-related effects on Mycobacterium leprae viability, followed by another 5 months of daily minocycline for overall efficacy and persistence of clinical and antibacterial effects. Subsequently, the patients were given standard WHO/MDT chemotherapy for multibacillary leprosy. Clinical improvement was recognizable during the first month, occurring much earlier among those on minocycline 200 mg daily than those who received minocycline 100 mg daily. A similar change also was observed in one patient 11 days after three daily doses of 100 mg of minocycline. At the end of 6 months, all patients were clinically improved with a slight reduction in the average bacterial index (BI) and logarithmic index of bacilli in biopsy (LIB). The effects of minocycline on viability by mouse foot pad inoculation and palmitic acid oxidation assays were noted beginning at 10 to 14 days of daily dosing and becoming more definite after 30 days of treatment. Both tests correlated fairly well. Doses of 200 mg daily did not appear to be more efficient than minocycline 100 daily. Phenolic glycolipid-I (PGL-I) antigen determinations done on some patients during the first month remained positive and did not correlate with changes in viability results. At the end of 6 months, after 5 months of 100 mg of minocycline monotherapy, no viable organisms could be demonstrated by mouse foot pad inoculation and palmitic acid oxidation assays; assays for PGL-I antigen were all negative.

Title:Epithelioid granuloma in the iris of a lepromatous leprosy patient; an unusual finding.

Abstract:This case report depicts a case of histopathologically confirmed polar lepromatous (LL) leprosy with a bacterial index of 4+. He experienced recurrent episodes of erythema nodosum leprosum (ENL) in the first 5 years after diagnosis. Skin smears became negative after 6 years of dapsone monotherapy and have remained negative since that time. At 23 years after diagnosis, the patient had developed cataracts and underwent intracapsular cataract extractions with broad-based iridectomies. In one of the iris specimens, histopathologic examination revealed a focal granuloma composed of epithelioid cells. Subsequently a lepromin skin test showed a positive Mitsuda reaction with a borderline tuberculoid histopathology. This clearly illustrates the immunological upgrading of a polar lepromatous patient, perceived first in the iris tissue. (AU)^ien.

Title:Pathology of a lepromatous eye.

Abstract:Histopathological examination of an enucleated eye from a lepromatous leprosy patient showed the cornea, ciliary body, and part of the choroid to be infiltrated by macrophages filled with Mycobacterium leprae. The walls of blood vessels in the sclera, ciliary body and the anterior choroid demonstrated the presence of M. leprae, giving credence to the blood-borne entry of M. leprae into the eye. Unlike the eyes of experimental animals infected with M. leprae, histopathological study of this eye from a lepromatous leprosy patient demonstrated that M. leprae, although demonstrable in the anterior choroid, could not be found in the posterior parts of the eye, substantiating the claim that leprosy does not affect the posterior parts of the eye directly. Title:Single lesion paucibacillary leprosy: baseline profile of the Brazilian Multicenter Cohort Study.

Abstract:In Brazil, there is little information about the clinical and epidemiological characteristics of paucibacillary, single skin lesion leprosy patients (SSL-PB). Only recently has the official notification system distinguished leprosy patients with a single lesion as a clinical entity, for whom the single-dose ROM (rifampin, ofloxacin and minocycline) regimen has been recommended. In this paper, we describe the baseline clinical features and the immunological background of a multicenter cohort of SSL-PB leprosy cases enrolled between December 1997-1998. Patients were recruited at health centers located in the following regions: Southeast = Rio de Janeiro; North = Amazon and Rondônia states and Center-West = Goiás state. Eligible cases were newly detected, untreated single-lesion leprosy patients without thickened nerve involvement, and were assessed by clinical, bacilloscopic and histopathological exams. The Mitsuda skin test and anti-PGL-I serology (ELISA) were also performed. Of the 299 SSL-PB leprosy patients, 259 (86.6%) fulfilled the criteria for single-dose ROM intervention. Our results showed that patients recruited from different sites had similar features, considering the clinical and immunological profiles. There was a predominance of adults (mean age 32.4; S.D. = 16.0), and a BCG scar was detected in 76.7% of the children (< or = 15 years old). Only 7 cases were diagnosed as the multibacillary type, representing less than 3% of the patients being misclassified. Our data indicate that in Brazil SSL-PB case ascertainment based on clinical and bacilloscopic criteria can be accurately defined under a routine control program; 75.0% of SSL-PB cases were Mitsuda positive (> or = 5 mm) and seropositivity for anti-PGL-I was detected in 17.3% of the patients. These data are compatible with effective cell-mediated immunity and low bacillary load, suggesting favorable clinical outcomes for most SSL-PB participants of this cohort. (AU)^ien.

Title:Cytokine profiles in paraffin-embedded biopsy samples of lepromatous leprosy patients: semi-quantitative measure of cytokine mRNA using RT-PCR.

Abstract:A reproducible technique for fixation of tissue, RNA extraction and reverse transcription polymerase chain reaction (RT-PCR) analysis from paraffin-embedded leprosy biopsies, has been developed and used to study the mRNA profiles. This approach is valuable in retrospective analysis of gene expression, and the handling of infectious biopsy material is also minimized. Among the methods of RNA extraction compared, the most efficient method was found to be incubation of the tissue sections in digestion buffer followed by extraction with Trizol. The experimental conditions were optimized for first strand cDNA synthesis and PCR, and used to measure the quantity of cytokine transcripts in biopsy materials. Interleukin-10 (IL-10) mRNA was detectable in all cases examined, which correlates well with other earlier reports using frozen tissues. However, IL-5 transcripts were present in 60% of the biopsies, unlike the earlier reports which showed IL-5 mRNA in all LL cases. Transforming growth factor-beta (TGF-beta) mRNA was detected in 80% of the biopsies, and this confirmed earlier immuno-cytochemical data which showed TGF-beta protein in all cases. Tumor necrosis factor-alpha mRNA was found in about 60% of the biopsies; whereas interferon gamma mRNA was detected in 30% of the LL cases. In conclusion, the results obtained in our study confirm and extend earlier observations which examined cytokines in peripheral blood cells and dermal lesions of leprosy. The simplicity of this method would allow the examination of a large number of samples across the spectrum of leprosy.(AU)^ien.

Abstract:Utilizando a técnica de Seymour Cohen, os autores fizeram a dosagem de polissacárides em 17 soros normais, 11 soros de portadores de lepra tuberculóide e 60 soros de lepromatosos entre os quais 23 soros de pacientes em diferentes fases de reação leprótica. Tecem ligeiros comentários acêrca da técnica empregada e das possíveis origens dos polissacárides circulantes. Concluem que os polissacárides séricos nos doentes de lepra apresentam-se ligeiramente aumentados em relação às pessoas normais, sendo pequena a diferença entre casos tuberculóides e lepromatosos. Entretanto, na reação leprótica, verifica-se aumento proporcional … intensidade do quadro sindrômico. (AU)^ipt.

Title:Aspectos epidemiológicos e clínicos da lepra em Uruguaiana.

Abstract:1) - A relação masculino-feminino nos doentes de lepra de Uruguaiana, foi de 1: 1,06 e na população ‚ de 1: 1; discreta elevação nos femininos. 2)- Os brancos contaminaram-se 5,1 mais que os mistos e 2,2 que os pretos, nos respectivos coeficientes. 3)- Maior coeficiente de incidência nos viúvos e o mais baixo em menores de 15 anos. 4)- O sexo feminino foi o mais acometido (52 para 49), embora as mulheres fizessem mais formas de resistência. (31 para 25 dos homens). 5)- Os estrangeiros apresentaram um coeficiente de incidência 5 vêzes maior do que os nacionais, porém tiveram 65% de formas de resistência. 6)- Os estrangeiros contribuiram com 17% dos casos de endemia, sendo em maior número os uruguaios, seguidos dos argentinos. 7)- A incidência da leprose, em geral acompanhou os tipos de trabalho da comunidade. 8)- Descobriram-se 33 fontes de contágio e em 67,4% das vêzes permaneceram ignoradas. 9)- Quando conhecido o foco transmissor. 88% foi encontrado na própria fam¡lia e 76% no mesmo domicílio. Predominou o contágio de pais para filhos com 36,5%, seguido do mesmo entre irmãos (24,4%) e de tio para sobrinhos (12%). 10)- Sòmente o tipo lepromatoso transmitiu a doença de Hansen, apesar de terem existido 21, das formas clínicas com baciloscopia positiva. 11)- O tempo de incubação da moléstia em 31 comunicantes que enfermaram foi de 5,4 anos. 12)- O tempo de doença quando fichado, variou de 1 mês a 18 anos, com média de 3 anos e 3 mêses. 13)- A idade média quando se inicou a moléstia nos 101 leprosos, foi de 40 anos e 4 mêses, provando tratar-se de tipo de infecção tardia. Predominou no grupo etério de 30 a 39 anos. em 13 contatos que adoeceram após o fichamento foi de 34,5 anos. 14)- A incidência da leprose apresentou-se 21 vêzes mais elevada nos maiores de 15 anos que nos menores...(AU) ^ipt.

Abstract:The author reports his work and other data about the correlation between tuberculosis and leprosy, considered under the immunoallergic, clinic and epidemiologic point of view. 1. Cosidering the immunoallergic correlation between tuberculosis and leprosy with all the data available, he says that in some materials there is an association between positive tuberculin and the positivity of lepromin. There are other elements that reduce the importance of the above mentioned association and of eventual consensibilization. New experiments should be done, especially out of urban areas and in children, in order to comprove that the paralellism of the tuberculin and lepromin curves depends of a relation of cause and effect and not of an association. 2. The researchs of Fernandez, Melson, Hadler and Ziti, and others, favor the hypothesis of transitory tuberculin sensibilization caused by the "M. leprae" and "m. lepraemurium". However, there are many data that reduce its importance. In case it come to be proved in other experiments it wil be difficult to apreciate correctly the correlation between the tuberculin and the lepromin test. When would the first influence to second of vice-versa? and in what degree? Even proved this cross sensibilization from "M. leprae" to "M. tuberculosis", the A. thinks that it would be of slight importance, without interfering largely in the specificity of the tuberculin test. 3. Tuberculosis influencing the clinical course of leprosy and vice versa: the author believes that the data are not enough to prove that tuberculosissnakes more favorable the clinical course of leprosy and vice versa. 4. Tuberculosis influencing the epidemiology of leprosy and vice versa: the author considers the Chaussinand's hypothesis that the erradication of leprosy in Europe was chiefly determined by presence of tuberculosis among the people.
